Compass offers peer support through meal sites and community outreach, along with more structured case management for those who need help navigating available services in the community.
Having someone to talk to and help you through the processes can make complicated systems easier and more manageable to navigate.
PEER SUPPORT THROUGH OUTREACH
Peer support offers a chance to connect with someone who can listen, answer questions and help you navigate the path forward, whatever that may look like for you.
You may meet a peer at a meal site, during an outreach event or elsewhere in the community.

A PEER MAY BE ABLE TO HELP YOU:
Schedule and prepare for appointments
Replace an identification card or request a birth certificate
Find community resources
Work through applications and forms

COMPASS OUTREACH
CASE MANAGEMENT IN COMPASS HOUSING
Case management provides more structured, ongoing support to anyone who needs it.
A case manager works with each participant to understand their circumstances, identify practical goals and work through the steps toward them.
That support may include help with benefits, housing applications, appointments, identification documents and connections to other providers.
The work is personal because every participant has different needs, responsibilities and hopes for what comes next.

COMPASS OUTREACH
CONNECTING WITH COMMUNITY CARE

Compass does not provide clinical mental health care or addiction treatment directly, but our team can help connect people with community organizations that offer behavioral health care, recovery services, medical care and other specialized support.
Community providers may also attend selected meal services and outreach events, making it easier to ask questions and connect with care in a familiar setting.

WHAT TO EXPECT
Peer support often begins with a conversation at a meal site or outreach event, although you can also call Compass to ask how to connect with someone. A peer can listen, help with a practical task or connect you with someone better placed to assist.
Case management is a working relationship for all participants in Compass programs. Participants meet with their case manager regularly to review goals, address barriers and coordinate the support they need for their individual needs.
